This document is used to grant permission for copyrighted materials to be used by another party, for compensation.

This document generates an agreement in which a person or entity grants permission to another person or entity to use copyrighted materials. 

Once completed, the Copyright Owner and a witness to the agreement must sign the agreement in the space provided.
